Ohio Shared Living
In REM Ohio’s Ohio Shared Living program, also known as host homes, adults with intellectual and developmental disabilities live in private family homes with dedicated caregivers who we call Mentors.
We use the term “Mentor” because the people who open their homes to these individuals become so much more than care providers. They are advocates, teachers, friends, and family.
Backed by our experienced human services professionals, our Mentors deliver personalized services to help individuals live life to the fullest. Our Ohio Shared Living program includes:

In partnership with our Mentors, the individuals, and their families we develop Individualized Service Plans (ISPs) and continually adjust services as individuals reach new goals or their needs change.
Successful Matches
In our Ohio Shared Living program we look at the needs, skills and personalities of individuals and Mentors to create a successful match—one that leads to a meaningful, long-term relationship. In addition to personalized support, our Mentors provide individuals with countless opportunities to take part in everyday activities of family life, such as:
